CPSI Demographic Survey Form

Please download the NRPA Demographic Survey. The candidate must bring the completed Survey with them to the exam check-in.  Your name, ID Number, contact information, mailing address, test date and testing location must be recorded in these spaces. The Test Center Number for this exam is 9363. The ID Number will be emailed to you in a separate email. The remaining sections are to be completed by the candidate.  All of this information must be filled in on each candidate’s survey. The exam score report will be mailed to the address on the survey.  Missing, inaccurate, or illegible information may result in a delay in reporting that candidate’s exam score.  All surveys must be reviewed prior to the exam for complete, legible mailing and email address information.  The surveys will be collected by the Proctors just before the exam begins.  They must be returned to PSI along with the testing materials.
